Organizational Structure

CAAP is a C-corporation that is wholly owned by physicians through purchase of one (1) share of Common stock at $100 Par Value in the organization. The resources of CAAP are made available to all physician members. Physicians may become a member by a vote of the stockholders through its Credentialing Committee and Board of Directors.

CAAP is governed by its Board of Directors, which is comprised of five Specialists and five Primary Care physicians, ensuring that all physicians are equally represented. Board members are elected by the full membership, and serve for a three-year period. The terms are staggered so that one-third of the Board is elected by the membership annually.

Applying for Membership

Application steps:

  1. Complete a Georgia Uniform Application for Credentialing.
  2. Sign the CAAP Physicians Service Agreement.
  3. Sign the CAAP Subscription and Shareholders Agreement to purchase 1 share of common stock at $100 Par Value.

Monthly CAAP membership dues:

Primary Care Physicians:  $100.00
Specialty Dues:   $125.00
